If you seek to request records from any of the providers listed below, your request for records must be served separately. Every treating facility must be served individually. The following information is provided to help record requesters determine where to request records. The providers named below treat or have treated patients at the following treating locations.

California Sports & Spine Center does not accept record requests by fax, email, or drop service. Every request for records must be mailed to our Process Center located at 1142 S. Diamond Bar Blvd PMB 310, Diamond Bar, CA 91765 —DROP SERVICE IS NOT AUTHORIZED.

The "Treating Doctor's Name" and the "Treating Location Address" must be specified in your request for records. Your Subpoena or HIPAA Authorization request must be directed to "California Sports & Spine Center".
